Full-Time Admissions Physical Therapist (Home Health Visits)

BAYADA Home Health Care is seeking a Full-Time Admissions Physical Therapist to provide OASIS Start of Care (SOC) visits for adult and geriatric clients throughout Essex County, NJ.

In this specialized admissions role, you'll focus on conducting comprehensive evaluations and establishing personalized care plans that help patients safely recover and thrive in the comfort of their own homes.

Responsibilities:

  • Complete OASIS Start of Care (SOC) admissions visits for home health patients throughout designated territories.
  • Perform comprehensive physical therapy evaluations, including assessments of mobility, strength, balance, gait, endurance, transfers, safety, and functional abilities.
  • Develop individualized treatment plans based on patient needs and physician orders.
  • Collaborate with nursing and interdisciplinary team members to coordinate patient care.
  • Educate patients, families, and caregivers to promote independence, safety, and optimal outcomes.
  • Accurately document clinical findings and services using electronic medical records and tablet technology.
  • Continuously evaluate patient progress and adjust care plans as appropriate.

Qualifications:

  • Current New Jersey Physical Therapist (PT) license.
  • Minimum of one year of recent experience as a Physical Therapist.
  • Prior Home Health experience required.
  • OASIS experience required.
  • HCHB experience strongly preferred.
  • Strong clinical, communication, and organizational skills.

Compensation:

$68.00-$75.00 per point, based on experience and qualifications.
