Psychosocial Director, Hospice

POSITION SUMMARY:

The Psychosocial Director provides leadership, consultation, and operational oversight to ensure the delivery, quality, and consistency of hospice psychosocial services across multiple sites. In partnership with the Division Director of Psychosocial, this role supports local psychosocial teams by promoting high‑quality patient and family care, advancing practice standards, strengthening staff development, and ensuring regulatory compliance throughout the hospice division.

MINIMUM QUALIFICATIONS:

  • Exemplifies characteristics of The BAYADA Way: compassion, excellence and reliability.
  • Master’s degree in Social Work (MSW) from an accredited program required. Master’s degrees in Divinity (M.Div) or Music Therapy may be considered with Division Director approval if:
    • Prior leadership experience in a comparable psychosocial or spiritual care role OR
    • Demonstrated capacity to fulfill psychosocial director responsibilities based on demonstrated performance within BAYADA.
  • Meets and maintains all applicable state licensure or certification requirements for their discipline (e.g., LCSW, MT-BC, chaplaincy certification).
  • Minimum three (3) years of people management experience in social work, music therapy, or pastoral care within hospice or palliative care. Program development experience preferred.
  • Minimum three (3) years of leadership experience in healthcare; hospice leadership strongly preferred.
  • Demonstrated excellence in interpersonal, written, and verbal communication.
  • Demonstrated skills in conflict resolution, family systems communication, and crisis intervention.
  • Demonstrated proficiency with electronic health record systems and documentation best practices.
  • Ability to work independently, prioritize responsibilities, and exercise sound clinical and administrative judgment.
  • Achieves a passing score on all required competency assessments relevant to the populations and services supervised.
  • Successful completion of criminal background screening and federal/state exclusion checks.

PREFERRED QUALIFICATIONS:

  • Demonstrate working knowledge of the CMS Hospice Conditions of Participation, particularly those related to psychosocial, spiritual, and bereavement services.
  • Experience supervising psychosocial disciplines within an interdisciplinary group (IDG), including chaplains, bereavement staff, and volunteers is strongly preferred.
  • Experience developing, implementing, or managing a hospice bereavement program, including familiarity with risk assessments, follow-up protocols, and community support resources.
  • Formal training or certification in trauma-informed care preferred.

PRIMARY RESPONSIBILITES:

  • Demonstrate and communicate the core values of BAYADA Hospice and The BAYADA Way.
  • Ensure compliance with Medicare Conditions of Participation (CoPs), state regulations, accreditation standards, and organizational policies related to psychosocial care.
  • Provide leadership, consultation, and operational oversight to ensure consistent, high-quality psychosocial, bereavement, and volunteer services including:
    • Directly manage Bereavement and/or Volunteer Coordinators.
    • Support implementation and advancement of psychosocial services across multiple hospice locations in partnership with the Division Director.
    • Partner with regional, market, and site leadership to promote consistent, high‑quality psychosocial care for patients, families, and caregivers.
    • Assist in promoting compliance with CoPs, state regulations, accreditation standards, and organizational policies related to psychosocial care.
    • Oversee the hospice bereavement program to ensure compliance with CoPs, timely risk assessments, and appropriate follow‑up services.
    • Support IDG effectiveness by promoting psychosocial integration in care planning, case review, and interdisciplinary communication.
    • Travel to hospice offices and service areas as needed to support program initiatives, team development, operational needs, and leadership partnership.
    • Develop and facilitate education, training, and resources to strengthen psychosocial practice, communication, caregiver support, and end-of-life care.
    • Participate in the Infection Control Program by identifying and reporting infections and risks.
    • Participate in the manager on‑call rotation.
    • Provide support to site leadership on complex psychosocial cases, care coordination challenges, patient/family escalations, and interdisciplinary collaboration.
    • Act as a client advocate by communicating and collaborating with team members regarding client status, care plans, and expected outcomes.
  • Ensure regulatory compliance, performance monitoring, and efficient psychosocial workflows including:
    • Assist with developing, implementing, and refining psychosocial workflows, standards, tools, and best practices across the hospice division.
    • Ensure documentation progresses efficiently from start of care through discharge and the bereavement period.
    • Support optimization of psychosocial documentation processes and EHR workflows to improve accuracy, efficiency, and compliance.
    • Work with field staff to ensure accurate and timely case management, authorization management, and interdisciplinary communication.
    • Analyze psychosocial and bereavement program data to identify trends, gaps, and opportunities for improvement.
    • Partner with Quality, Compliance, Education, and Clinical Operations teams to support regulatory readiness, audits, policy implementation, and quality improvement initiatives.
    • Consult with site Directors to monitor BAYADA’s KPIs and BAYADA Process Measures.
    • Collaborate with local leaders to monitor psychosocial operations, staffing needs, caseload trends, productivity, and service delivery.
    • Work with office staff to respond to ADRs and denials.
  • Provide clinical oversight to psychosocial field staff:
    • Participate in hiring and interviewing of psychosocial field staff.
    • Observe and confirm competency of psychosocial field staff.
    • Maintain knowledge of productivity requirements and ensure full‑time and guaranteed staff meet expectations.
    • Participate in new hire and client‑specific orientations related to care, safety, procedures, documentation, and BAYADA policies, both in the office and in the home.
    • Supervise psychosocial field staff through home visit observation and documentation review.
    • Manage performance of direct reports including recognition, coaching, counseling, and termination of employment, when necessary.
    • Complete regular performance evaluations with input from the Service Office leadership.
    • Provide leadership in identifying, escalating, and managing high-risk psychosocial situations to ensure patient and staff safety.
    • Participate in ongoing education and professional development of field staff.
    • Serve as a liaison between field and office staff.
  • Support the office and company team:
    • Provide clinical guidance and judgment to office colleagues to support understanding of client needs.
    • Assist with administrative activities such as filing and office upkeep.
    • Represent BAYADA in community and marketing activities, including visits to physician offices and facilities.
    • Participate in Quality Improvement activities.
    • Participate in office and company meetings, events, and functions.
  • Demonstrate solid performance or exceed performance standards in key job dimensions/attributes as defined on the Performance Appraisal for Office Staff (0-3240).
  • Perform related duties as required or requested by Supervisor.

OTHER RESPONSIBILITIES:

  • Together with the office staff, answer phones, greet visitors, and generate correspondence as needed.
  • Maintaining current knowledge of standards of practice, BAYADA policies, and regulatory requirements (government, contract, accrediting body).
  • Pursuing ongoing professional growth through continuing education and/or organizational membership.
  • Fill in to support field staff when staffing is unavailable.
