BAYADA Home Health Care is seeking a RN Nurse Case/Clinical Manager for our St. Cloud Pediatrics (SCP) office in Minnesota. This office provides adult and pediatric skilled shifts as well as pediatric personal care services.

Our Skilled Nursing practice provides expert, compassionate care to medically fragile children and adults in the home, including clients with complex needs such as tracheostomy and ventilator support, seizure management, and tube feedings.

Position Summary

The Clinical Manager leads field staff in delivering excellent client service, clinical care, quality, and adherence to policies and procedures. This role is also accountable for internal case management and field staff evaluation and development.

Responsibilities

  • Lead, coach, and support field staff to ensure excellent client service, clinical care, quality, and policy adherence.

  • Conduct client assessments and complete the care planning process.

  • Manage clinical care and case management responsibilities across the office's client population.

  • Provide clinical oversight, supervision, and development of field employees.

  • Review field employee documentation and clinical performance, and provide timely coaching and follow-up.

  • Support continuity of care for adult and pediatric skilled clients, including pediatric personal care services.

  • Partner with office leadership and interdisciplinary team members to ensure safe, high-quality care delivery.

  • Participate in after-hours clinical consultation and on-call support as needed.

  • Reinforce The BAYADA Way through compassion, excellence, and reliability in daily leadership and decision-making.

Qualifications

  • Current Registered Nurse license in good standing in Minnesota required.

  • Minimum of two years of nursing experience preferred; home health experience may vary based on office need.

  • Experience supervising, mentoring, or developing clinicians in a home care, pediatric, or skilled nursing setting strongly preferred.

  • Strong clinical assessment skills and the ability to support complex pediatric and adult skilled cases.

  • Experience with medically fragile children and/or adults strongly preferred, including trach/vent, seizure management, and tube feeding cases.

  • Trach, vent, and tube-feeding experience in a home health care setting preferred, along with the ability to teach and train other clinicians.

  • Demonstrated leadership, organization, multitasking, communication, and problem-solving skills.

  • Commitment to professional, ethical, and safe clinical practice.
