BAYADA Home Health is seeking a compassionate and experienced Physical Therapist (PT) to provide one-on-one home health visits to adult and senior clients throughout Monmouth, Somerset, and Union Counties, New Jersey.

Responsibilities

As a Home Health Physical Therapist, you will:

  • Provide skilled physical therapy services to adult and geriatric patients in their homes
  • Complete comprehensive physical therapy evaluations, including assessments of:
    • Joint mobility and range of motion
    • Muscle strength and endurance
    • Balance, gait, and functional mobility
    • Transfers and ambulation
    • Home safety and fall risk
  • Develop and implement individualized treatment plans designed to maximize function, mobility, and independence
  • Provide therapeutic exercise, manual therapy, gait training, balance training, and other evidence-based interventions
  • Implement appropriate interventions to promote skin integrity and support wound prevention and healing
  • Collaborate with nursing and the interdisciplinary care team to coordinate patient care
  • Educate patients and caregivers on home exercise programs, fall prevention, safe mobility techniques, and equipment use
  • Reassess patient progress and modify treatment plans as appropriate
  • Complete accurate and timely documentation using an electronic medical record (EMR) system

Qualifications

  • Current Physical Therapist (PT) license in the State of New Jersey
  • Minimum of one year of recent Physical Therapist experience
  • Graduate of an accredited Physical Therapy program approved by the American Physical Therapy Association (APTA) or equivalent accrediting body
  • Home Health experience preferred, but not required
  • Strong clinical assessment, communication, organizational, and critical thinking skills
  • Comfortable using electronic medical records (EMR)
  • Ability to work independently while collaborating with an interdisciplinary care team
  • Reliable transportation and willingness to travel throughout Monmouth, Somerset, and Union Counties

Additional Information

Base Pay, depending upon qualifications, status, and subject to negotiation:

Full-Time

$75.00–$80.00 per point ($117,000- $124,800 annual earning potential)

Per Diem

$75.00–$80.00 per point

$10,000 Sign-On Bonus for Full-Time PTs Only
