Physical Therapist (PT) – Senior Living Visits

Full-Time or Part-Time | Burlington & Camden Counties, NJ

BAYADA Home Health Care is seeking an experienced Physical Therapist (PT) to join our Senior Living team serving Burlington and Camden Counties, NJ.

In this role, you'll provide one-on-one physical therapy services to adult and geriatric clients in independent living, assisted living, and residential group home settings. You'll help clients regain strength, improve mobility, maximize independence, and safely remain in the place they call home.

Responsibilities

  • Provide skilled physical therapy evaluations and treatment during home and community visits.
  • Assess mobility, gait, balance, strength, range of motion, transfers, endurance, and overall functional status.
  • Develop and implement individualized treatment plans using evidence-based interventions, including therapeutic exercise, gait training, balance training, and manual therapy techniques.
  • Perform skin integrity assessments and implement interventions to help prevent wound development.
  • Continuously evaluate patient progress and modify treatment plans to achieve optimal outcomes.
  • Collaborate with nursing and other interdisciplinary team members to coordinate comprehensive patient care.
  • Educate clients, families, and caregivers on exercises, safety strategies, mobility techniques, and equipment use to promote independence.
  • Complete timely and accurate documentation using BAYADA's electronic medical record (EMR) system.

Qualifications

  • Current New Jersey Physical Therapist (PT) license.
  • Graduate of an accredited Physical Therapy program approved by the American Physical Therapy Association (APTA) or equivalent accrediting body.
  • Ability to work independently and effectively manage your schedule.
  • Excellent communication, organizational, and problem-solving skills.
  • Experience with electronic medical records (EMR) preferred.

Preferred Experience

  • Dementia Care Certification.
  • Experience working with adults with intellectual and developmental disabilities (IDD).
  • Experience caring for clients with Alzheimer's disease or other forms of dementia.
  • Experience assessing Hoyer lifts, mechanical lifts, and durable medical equipment (DME), including writing letters of medical necessity.

Additional Information:

Base Pay, depending upon qualifications, status, and subject to negotiation:
$63.00-$73.00 per point ($98,280-$113,880 annual earning potential)
