As part of BAYADA's Transitional Care Program, you will play an essential role in reducing readmissions, improving outcomes, and ensuring a seamless transition for medically complex clients.

BAYADA Home Health Care is currently seeking an experienced Registered Nurse (RN) to fill the position of Transitional Care Manager (TCM) supporting our Adult and Pediatric Skilled clients in MA,NY, NH, and RI.

Territory: Massachusetts, New Hampshire, Rhode Island, and New York. Travel throughout these states is required.

The Transitional Care Manager will:

  • Develop and maintain strong relationships with hospitals, skilled nursing facilities, physicians, case managers, discharge planners, and other referral sources.
  • Educate referral partners, clients, and families on BAYADA's home health care services and the benefits of transitioning care to the home setting.
  • Facilitate safe and effective transitions of medically complex and chronically ill clients from facility-based care to home care.
  • Review medical documentation and collaborate with physicians and healthcare teams to assess eligibility for home health services.
  • Coordinate communication between referral sources and BAYADA service offices to ensure successful admissions and continuity of care.
  • Provide post-transition follow-up support to help improve patient outcomes and reduce avoidable hospital readmissions.
  • Create and execute strategic referral development plans to grow market presence throughout the territory.
  • Represent BAYADA at healthcare facilities, professional events, and community outreach activities.
  • Collaborate with operational and clinical leaders to identify business development opportunities and drive referral growth.
  • Track referral activity, maintain accurate records, and provide regular reporting on territory performance.

Qualifications for a Transitional Care Manager:

  • Current, unrestricted Registered Nurse (RN) license.
  • Bachelor's degree preferred.
  • Minimum of five (5) years of nursing, discharge planning, home health, critical care, long-term care, care management, or related clinical experience.
  • Previous experience in critical care, acute care, long-term care, discharge planning, case management, or transitional care strongly preferred.
  • Demonstrated success building relationships with healthcare professionals and referral sources.
  • Strong communication, presentation, and interpersonal skills.
  • Ability to work independently, manage a large geographic territory, and prioritize multiple responsibilities.
  • Strong clinical assessment and decision-making skills.
  • Proficiency with Microsoft Office applications and technology platforms.
  • Ability to travel extensively throughout New England, Primarily throughout Boston and Worcester weekly, and to RI, NH, and NY on a monthly basis.

BAYADA Offers:

  • Base Salary: $95,000 - $110,000 annually, based on experience and qualifications.
