BAYADA Home Health Care is seeking a Full-Time Physical Therapist (PT) to provide one-on-one home health visits to adult and geriatric clients throughout Burlington County, NJ.
As a BAYADA Physical Therapist, you'll help clients regain strength, improve mobility, increase independence, and achieve their personal goals—all in the comfort of their own homes. You'll have the autonomy and flexibility of home health while being supported by an experienced interdisciplinary clinical team.

How You’ll Make an Impact:
- Provide one-on-one physical therapy visits to clients in their homes throughout Burlington County.
- Perform comprehensive physical therapy evaluations and assessments of mobility, strength, balance, gait, endurance, transfers, safety, and functional abilities.
- Develop and implement individualized plans of care based on client needs, goals, and physician orders.
- Provide skilled physical therapy interventions, including therapeutic exercise, gait and balance training, mobility training, and other appropriate treatments.
- Help clients improve strength, mobility, safety, and functional independence in their home environment.
- Monitor client progress and adjust treatment plans as appropriate.
- Educate clients, families, and caregivers on therapeutic exercises, mobility techniques, fall prevention, safety strategies, and ongoing care.
- Collaborate with nurses, therapists, physicians, and other members of the interdisciplinary care team.
- Complete timely and accurate clinical documentation using BAYADA's electronic medical record (EMR) system.
Qualifications:
- Current New Jersey Physical Therapist (PT) license.
- Graduate of an accredited Physical Therapy program.
- Minimum of one year of recent experience as a Physical Therapist.
- Home health experience preferred, but not required.
- Strong clinical assessment, communication, interpersonal, and organizational skills.
- Ability to work independently and effectively manage time and schedule.
- Prior experience with electronic medical records (EMR) preferred.
- Reliable transportation and ability to travel to client homes throughout Burlington County.

Additional Information:
Base Pay, depending upon qualifications, status, and subject to negotiation:
$63.00-$73.00 per point
