Salary: $48.00 per hour

BAYADA HOME HEALTH CARE is seeking a Registered Nurse (RN) Clinical Manager for our Essex Assistive Care State Programs (ESS) office in Bloomfield, NJ. In this part-time RN leadership role, you will provide clinical oversight for clients receiving state-funded personal care services while supporting and developing a team of Home Health Aides and Personal Care Assistants.

Our Assistive Care State Programs services help adults and seniors receive personal care and support at home through Medicaid waiver and other state-funded programs. These services promote safety, independence, and quality of life for clients who may otherwise require a higher level of care or institutional support.

What you’ll do

• Provide clinical oversight for clients enrolled in state-funded Medicaid waiver and personal care programs
• Conduct client assessments and develop individualized care plans aligned with client needs and state program guidelines
• Supervise, support, and provide clinical guidance to Home Health Aides and Personal Care Assistants
• Ensure care delivery aligns with approved services, authorized care hours, state program requirements, and BAYADA standards
• Coordinate with state case managers, Medicaid managed care organizations, families, caregivers, and other external stakeholders
• Support client reauthorization processes and help ensure care hours remain aligned with approved program parameters
• Review documentation for accuracy, completeness, timeliness, and compliance with applicable state and program requirements
• Facilitate caregiver training, orientation, and competency documentation as required
• Monitor quality of care and respond to incidents, complaints, changes in client condition, and service concerns
• Conduct supervisory visits and follow up on client and caregiver needs
• Serve as a clinical resource, client advocate, and partner to the Director and Client Services Manager
• Participate in quality assurance, performance improvement, and audit-readiness activities
• Build positive relationships with clients, families, caregivers, state partners, and community resources

What you bring

Required

• Current Registered Nurse (RN) license in good standing in New Jersey
• Minimum of 2 years of RN experience
• Strong organization, documentation, communication, and problem-solving skills
• Ability to manage state program requirements, client needs, and competing priorities
• Ability to provide clinical leadership and support to caregivers in a community-based setting
• Comfort using computer systems for documentation, communication, and care coordination
• Commitment to serving clients and families with compassion, accountability, and respect

Preferred

• Experience with Medicaid, Medicaid waiver programs, or other state-funded services
• Home care, Assistive Care, or community-based nursing experience
• Prior supervisory, leadership, or caregiver-management experience
• Knowledge of state-specific personal care program rules, authorizations, and documentation standards
• Experience coordinating with state case managers or managed care organizations
• Experience with caregiver training, competency validation, or quality oversight
• Passion for serving underserved clients and helping people remain safely at home
