Rehabilitation Manager – Home Health- Senior Living | Wilmington, MA
Salary: $100,000/year depending on qualifications
Schedule: Full-time, exempt | Monday–Friday | 8:30 a.m.–5:00 p.m. | Flexibility for occasional extended hours and travel throughout the service area as needed

BAYADA HOME HEALTH CARE is seeking a Rehabilitation Manager for our Wilmington, MA Senior Living Office. In this rehabilitation leadership role, you will help oversee therapy services while supporting a team of physical, occupational, and speech-language therapists who help clients recover, regain function, and remain safely at home.
BAYADA’s Senior Living services bring skilled nursing and therapy directly to clients in their community setting. Our rehabilitation professionals support adults and seniors recovering from surgery, illness, or injury, as well as clients managing chronic conditions, with a focus on strength, mobility, communication, swallowing, safety, and independence.

What you’ll do
- Lead, mentor, and support a team of physical, occupational, and speech-language therapists (potentially Nursing staff as well)
- Provide clinical and operational oversight for home-based rehabilitation services throughout Wilmington, MA and surrounding areas.
- Support high-quality, coordinated therapy care from referral through discharge
- Partner with therapists to promote individualized treatment planning and meaningful client outcomes
- Support care coordination among therapists, nurses, physicians, clients, families, and other members of the care team
- Review documentation, care plans, and service delivery for accuracy, timeliness, quality, and compliance
- Monitor client progress, satisfaction, continuity of care, and therapy-related service needs
- Facilitate orientation, onboarding, competency development, and ongoing education for rehabilitation staff
- Provide coaching, feedback, and performance support to therapists
- Participate in hiring, interviewing, and workforce planning for rehabilitation positions
- Partner with the Director and Clinical Manager on staffing, scheduling, referrals, service delivery, and office performance
- Support quality assurance, performance improvement, and regulatory-readiness activities
- Build strong relationships with physicians, hospitals, facilities, referral sources, clients, families, and community partners
- Represent BAYADA in the community and support growth through excellent service and reputation

What you bring
Required
- Current professional license or credential in an applicable rehabilitation discipline, such as Physical Therapy or Occupational Therapy.
- Experience providing rehabilitation services in home health, community-based care, or a related clinical setting
- Strong clinical judgment, organization, communication, and problem-solving skills
- Ability to support coordinated, outcomes-focused care across an interdisciplinary team
- Comfort using electronic documentation and communication systems
- Ability to travel throughout Wilmington, MA and surrounding communities as needed
- Commitment to compassionate, client-centered care and professional excellence
Preferred
- Prior rehabilitation leadership, supervisory, or team-lead experience
- Home Health experience
- Experience leading or supporting PT, OT, and speech-language therapy teams (possible RNs and LPNs)
- Experience with care coordination, clinical documentation review, quality improvement, or regulatory compliance
- Experience mentoring, coaching, and developing rehabilitation clinicians
- Strong relationships with local healthcare providers, facilities, and community referral partners
